• I am not suffering from fatigue.
• I am not impaired by prescription or over the counter medication.
• I am not under the influence of alcohol or drugs.
• I have taken the statutory minimum rest breaks during the last 24 and 72 hours.
• I have had a minimum of seven continuous hours stationary rest since completion of the previous shift.
• I have sufficient driving hours available under the relevant fatigue management regime (standard, BFM, AFM) to perform the task including a reasonable allowance for delays.
• I am free from any other factor which may prevent me from safely completing this task.

If you are not fit to drive, do not drive. Let your manager know and you can work together to manage the issue.

• In-cab check completed (seat, warning lights, horn, gauges, air system, washers and wipers, windows and mirrors, loose items restrained, paperwork, headlights and hazard lights).
• Truck and trailer walk-around completed — nothing out of the ordinary observed.
• Every wheel and tyre checked — sufficient tread, free from damage or unusual wear, wheel nuts present and indicators lined up.
• Load restraint equipment checked and free from damage; load adequately restrained as per NTC Load Restraint Guide.
• All air tanks drained — water not excessive and not excessively oily.
• All couplings securely coupled, undamaged, lubricated, no fretting; trailer electrical and air lines connected and undamaged.
• All lights functional, secure and undamaged.
• No fluid or air leaks; no damage to vehicle or trailers; vehicle sitting normally on its suspension.

If you find any issues during the inspection, record them on the Vehicle Defect Report and report the issue to management.

• My heavy vehicle driver license is current and is compliant with the class of vehicle being driven.
• I am aware of suitable rest facilities and amenities (if applicable) on this route.
• All duties will be completed in accordance with regulations.
• I have had the legally required rest breaks before commencing this trip.
• The load is adequately restrained in all directions as per NTC load restraint guide and requirements.
• My work diary has been completed in accordance with regulations.
• I am free from effects of drugs and alcohol that would impair my ability to work / drive.
• I have completed a pre-trip equipment inspection and appropriate action has been taken before commencement of this trip.
• If this task takes me past 24 hours, I will consider what work time is available according to the scheme that the driver is accredited.
• The above stated vehicle has not and will not be modified in any way as to permit a breach of speed limits and I will not drive in breach of any speed limits.
• I will report any unscheduled rest breaks, mechanical failures, hazards or incidents which will impact this journey to Neohaul management.
